High-need patients require the most attention from health care providers and the health care system, with the top 1% of patients taking up more than 20% of health care expenditures. We will examine what policies are needed to optimize the care of such patients and whether systems can be designed to better serve their needs. What actions can support the spread and scale of evidence-based programs?
